Overview:
Following a tragic incident involving two fatalities during pipeline pressure testing works in Al Kamil, CID Gulf was appointed by APSR to lead a structured technical investigation. The aim was to determine the engineering and procedural failures that led to the collapse of a 3.5-ton concrete thrust block during unauthorized lifting operations by a subcontractor under NWS’s pipeline contract.
Scope of Work:
Site visit and data collection from Nama Water Services (NWS) and the main contractor Analysis of hydrotest method statements and lifting plans Review of safety protocols, site instructions, and permit-to-work systems Interview framework for subcontractor and NWS supervisory staff Engineering commentary on thrust block stability, trench safety, and equipment suitability Evaluation of NWS’s contract structure, SLA, and HSE response procedures Final report with root cause analysis, lessons learned, and preventive measures Optional roadmap for secondary investigation into NWS operational compliance
Outcome:
CID Gulf’s advisory support provided APSR with a technically sound, impartial assessment of the fatality, shedding light on gaps in site safety, contractor oversight, and emergency response. The investigation’s conclusions contributed to national safety reform discussions in water utility infrastructure.
